MRC Volunteer Application
The application can be mailed, emailed, or dropped off at the Framingham Board of Health, 150 Concord St, Room 221.  The law requires that all MRC Volunteers also sign a CORI (Criminal Offender Record Information). Please print and return BOTH forms. Your photograph will be taken for your badge when the completed forms are returned.
